To become authorized to haul loads for Tandem Transport, please complete the Brokerage Questionnaire / Application below.

It can be completed on line and submitted to Tandem Transport. It can also be printed, completed and mailed or faxed to us at 803 286-4144.

Upon receipt and approval of your application, Tandem will fax our package to you.

In order to complete your file, we will need a signed Broker / Carrier Agreement (in our package) along with your operating authority, insurance certificate and W-9. You will then be ready to haul loads for Tandem Transport Co., Inc.
